Please fill in the application form as fully as possible. Note that all of the fields are 'required' (red asterisk *) as they are important for us to deliver the best possible course for you. We have kept the information to a minimum. Thank you for your cooperation.

We need this information for your invoice.
Please grade your own level of English between 0 and 10, where 0 = complete beginner and 10 = native English speaker. This is only for us to use as a guide.

Online Terms & Conditions

Before you submit your application please read these terms & conditions :

  • If you want to cancel a lesson, this must be done by email so that we have a record of the time you cancelled and the reason you gave. Please email your teacher and copy in: adminteam@worcester-medical.com
  • Lessons that are cancelled less than 4 hours before the appointed time will be lost.
  • Lessons that are cancelled more than 4 hours before the appointed time can be re-scheduled directly with your teacher.
  • Blocks of 4-week lessons must be completed within 5 weeks of the first lesson that is taken.
  • Blocks of 8-week lessons must be completed within 9 weeks of the first lesson that is taken.
  • Continuation blocks of lessons must be booked and paid for in advance via: adminteam@worcester-medical.com
